It's spiritual, if you want it to be.Gotta remember, Tool lyrics are ambiguous as can be, so our innate need for validation and self service will form the meaning to be whatever we need it to be at the time.I think,objectively,that it's a song about the fine line between being an upstanding, peaceful citizen of the world, and just saying "Fuck the restraints; I've been pissed off for a long time and for a good reason,and will now run amok"--"the patient", as in patience.
